"Clear cell sarcoma of soft tissue (CCSST) is extremely rare, accounting for about 1% of all soft tissue tumors. As an invasive sarcoma, CCS has a high proportion of metastasis. Regardless of the pathological condition of the resected tumor, MRI or CT should be used every 1-2 years to perform recurrence checks on the primary site and metastasis screen in the lungs, liver, and bones. In this article,we focus specifically on the important role of medical imaging , especially MRI, in the CCS diagnosis, surgical guidance, recurrence evaluation, and metastasis detection. "1. Research Contribution This article documents a rare and life-threatening case of acute non-ST segment elevation myocardial infarction (NSTEMI) complicated by sudden ventricular free wall rupture (FWR) leading to cardiac tamponade. The successful emergency surgical intervention, which included open-heart repair and coronary artery bypass grafting, highlights a significant advance in the management of such critical complications. The report not only contributes to the limited literature on NSTEMI-associated FWR but also underscores the importance of rapid diagnosis and multidisciplinary management in achieving favorable outcomes. 2. Key Findings and Innovations The case study revealed the following: • Prompt diagnosis of cardiac tamponade and immediate pericardiocentesis were instrumental in stabilizing the patient before surgical intervention. • The use of multidisciplinary strategies, including real-time imaging, blood pressure management, and high-dose vasopressor medications, exemplifies a protocol that can serve as a reference for similar emergency scenarios. • This case demonstrates the feasibility of combining emergency open-heart repair with coronary artery bypass grafting to manage mechanical complications of myocardial infarction, particularly in rare NSTEMI cases. 3. Limitations As a single-case report, this study is inherently limited in its generalizability. The findings need to be validated through larger cohort studies or clinical trials. Additionally, the specific factors contributing to the rarity and mechanism of FWR in NSTEMI patients require further investigation. 4. Implications for Future Research This case highlights several directions for future research: • Developing predictive models to identify high-risk NSTEMI patients for cardiac rupture. • Evaluating the role of advanced imaging techniques in the early detection of myocardial wall weakening. • Investigating the long-term outcomes of patients undergoing emergency interventions for cardiac rupture to optimize post-surgical care. 5. Impact on the Field This article provides critical insights into the rare but fatal complication of FWR in NSTEMI patients. By documenting a successful intervention pathway, the study serves as an educational resource for clinicians and reinforces the necessity of timely surgical intervention in similar cases. Furthermore, it emphasizes the importance of integrating diagnostic precision with rapid therapeutic responses to improve patient survival rates. This case report adds valuable knowledge to the field of cardiovascular emergency care, particularly concerning rare NSTEMI complications. It underscores the potential of rapid, multidisciplinary interventions to achieve positive outcomes, even in life-threatening scenarios.
